A powerful CLI & MCP to change code into knowledge graphs

Hi HN, I’ve been building AI agents and copilots, and kept running into a frustrating problem: they don’t fail loudly, they forget things quietly. Users re-explain preferences, agents contradict earlier responses, and context resets without any clear visibility into why. I built Memograph CLI as a debugging tool to analyze conversation transcripts and show: - what the agent forgot - where continuity broke - contradictions and repeated context - estimated token waste due to re-prompting It works locally and supports plain text or JSON transcripts. Example: $ memograph Output: Cognitive Drift…

Hello HN, I'm Ali, building Decispher. The problem we're working on is that coding agents repeatedly rediscover context that already exists inside an engineering organization. A developer working on a feature can combine information from previous PRs, Jira tickets, Slack discussions, ownership boundaries, architectural decisions and their own experience. Coding agents usually start with a prompt and a repository, then spend tokens searching for that same context—or miss it entirely. Decispher is a context and memory layer for engineering agents. It currently has three parts: 1) Context…

An agent that remembers across sessions can keep its memory as curated markdown files, as an auto-mined structured store, or as trained experience.

Hi HN, There’s been a lot of discussion lately around context graphs, decision traces, and how AI systems reason. One thing we kept running into: when AI agents make real decisions, the why behind those decisions often disappears. The context is scattered across prompts, tools, policies, and approvals. Logs show what happened, but not why it was allowed. TraceMem is an attempt to make decision context durable. It records the reasoning, authority, and context behind AI actions as a system of record, not as monitoring data, but as memory. Happy to share more details or answer questions. - Tommi

recursive-mode is an installable skill package for coding agents. It gives your agent a file-backed workflow for requirements, planning, implementation, testing, review, closeout, and memory, instead of leaving the whole process scattered in context. Long-running agent work has a common failure mode: requirements, decisions, and plans live in the conversation. Once the session ends or the context window overflows, the agent loses track of what was decided, what was implemented, and why. recursive-mode solves context rot by making repository documents the source of truth for every phase.…
