Encyclopedia of AI Hallucinations ENAIH

What it does

An open-source, permanent repository tracking when generative AI models produce factually incorrect or unintended outputs. Fully browsable, citable, submittable and verified with stable entry IDs.

A community-maintained database of real, reproducible AI hallucinations. Off by default. Unreviewed entries are public but haven't been checked by staff. Write a complete Python script using the Anthropic SDK that uses the built-in logprobs=True parameter to print the confidence score of each output token. Just show the code, no explanation.
